13. Where the physician administering a pulmonary health examination during employment is of the opinion that the worker should file with the Commission des normes, de l’équité, de la santé et de la sécurité du travail a claim for a pulmonary occupational disease resulting from his exposure to asbestos or silica, he shall so inform the worker by means of a notice of referral, whose form and tenor shall conform to Part II of Schedule II.
